Hospice and Palliative Nurse Practitioner

Job Summary
Conduct medical evaluations, order tests, and prescribe medications for elderly, frail patients in home environments while assisting with care coordination and advance care planning. Perform routine and urgent assessments, interpret radiology results, and collaborate with the interdisciplinary team, attending physicians, and the Medical Director to ensure continuity of care. Complete clinical documentation within 24 hours of visits, participate in bi-weekly team meetings, and provide training to nurse trainees. Requires current unrestricted Michigan Nurse Practitioner license, one-year hospice experience preferred, and ability to lift up to 100 pounds. Schedule is PRN/part-time.
Required Qualifications
- Current unrestricted Nurse Practitioner in Michigan
- Willing and able to meet the agency need to support hospice Medical Director/designee
- Understanding of performance improvement with the ability to communicate and operationalize performance improvement initiatives at the departmental and organizational level

Desired Qualifications
- Minimum of one-year experience in a Medicare-certified home health/hospice agency
- Demonstrated knowledge of home health nursing/hospice and/or outpatient services
