Hospice Aide (Stillwater)

Job Summary
Deliver compassionate in-home hospice care as a Certified Home Health Aide/Hospice Aide under RN delegation. Perform nursing visits per the Plan of Care, document medication regimens, and update the medication profile. Communicate significant findings to the RN Case Manager, physician, and care team; report unsafe conditions and implement patient safety measures. Assist patients with self-care techniques and contribute to revisions in the Plan of Care as needed. Collaborate with the RN Case Manager during normal business hours for guidance. Requires HS diploma or GED, 1 year of hospice aide/ nursing assistant experience, a state hospice aide training program meeting 42 CFR §484.36, ability to travel up to 50%, and a valid driver’s license with reliable transportation. Required Qualifications
- High School Diploma or GED equivalent
- 1 year of experience as a Hospice Aide or Nursing Assistant
- State-established hospice aide training program meeting 42 CFR §484.36 and a competency evaluation program
- Experience in a home health environment preferred
- 50% travel required
- Valid driver’s license and insurance and reliable transportation to perform job tasks
