Job Summary
Provide direct personal care for hospice patients, including bathing, dressing, oral hygiene, and mobility assistance. Monitor patient conditions, report changes to the RN Case Manager, and administer routine skin care and medication reminders. Maintain a clean environment, use equipment safely, and support family caregivers emotionally. Adhere to all documentation and HIPAA compliance standards while participating in quality assessments. Requires an active CNA license, valid driver's license, and ability to lift up to 50 pounds. Based in a home setting with an hourly base range of $20-27.
Required Qualifications
- active Certified Nursing Assistant (CNA) license in the state of practice
- at least 18 years of age
- ability to pass background study, consumer report and drug test
- Valid State Driver's License
- access to an automobile in good working order
- automobile insurance in accordance with state and organizational requirements
- an acceptable driving record
- Pushing/Pulling and Lifting/carrying up to 30 pounds
- Occasionally up to 50 pounds
- ability to adapt to a variety of work conditions to ensure the highest quality of patient care in a home setting
