Job Summary
Provide personal care including bathing, skin/nail care, oral hygiene, and dressing/undressing to patients in their homes. Record vital signs, report observations to the care team, assist with meal planning and feeding, and maintain accurate documentation of services provided. Travel to patient homes throughout the day to deliver hands-on support under the direction of a Registered Nurse. Promote patient comfort, dignity, and safety while ensuring the highest possible quality of life at the end of life. Full-time schedule from 8:00am to 5:00pm with rotational weekends.
Required Qualifications
- Current CNA certification in the state of practice
- Valid driver's license, clean driving record, and auto insurance
- Travel to patient homes throughout the day and provide care as scheduled
