Job Summary
Hospice Aide position (full-time, 40 hours/week) within the PVHPC Hospice & Palliative Care team. Provides direct patient care under the interdisciplinary team, assisting with personal care, meal preparation and nutritional support, maintaining a safe, clean environment, and reporting changes in patient condition to the RN/case manager. Works per plan of care, may assist with additional procedures with training, and participates in IDT meetings. Travel is required; on-call rotation including weekends/holidays is expected. Requires CNA license, CPR certification, and Massachusetts Nursing Assistant certification; CHPNA preferred. Required Qualifications
- High school diploma or equivalent
- Satisfactory completion of training program approved by MDPH for HCA-III and other regulators
- Possesses and maintains current CPR Certification
- Massachusetts Nursing Assistant certification required
- CHPNA Certified Hospice and Palliative Nursing Assistant preferred
- Valid driver’s license
- Minimum of one year CNA experience; Home Health or Hospice experience preferred
