Job Summary
Provide individualized hospice care to patients in their homes, visiting one-on-one to assist with activities of daily living including bathing, feeding, dressing, and mobility exercises. Deliver personal care services such as oral care, skin care, and incontinence management while maintaining clean living areas. Monitor vital signs like temperature and blood pressure, prepare meals, and complete accurate documentation of all services provided. Maintain independence in the field to travel within the West Twin Cities Metro area, requiring a valid driver's license and reliable transportation.
Required Qualifications
- Certified Nursing Assistant on the MN Nursing Assistant Registry
- One year experience as a Home Health Aide
- Able to travel within geographic area serviced by the office
- Valid driver's license
- Reliable transportation
- Proof of auto insurance
- CPR/BLS Certification
- Must be active and in good standing on the MN Nursing Assistant Registry
