Job Summary
Provide direct patient care under RN supervision, including assistance with bathing, grooming, oral hygiene, skin care, nail care, and other personal needs. Support patients with activities of daily living (ADLs) to promote dignity, safety, and comfort. Safely assist with transfers and mobility using equipment such as hoyer lifts, gait belts, wheelchairs, walkers, canes, and crutches. Report observations and changes in patients' physical or emotional condition to the RN Case Manager and interdisciplinary team. Offer emotional support and compassionate presence to patients and caregivers while respecting professional boundaries in homes and healthcare facilities.
Required Qualifications
- Current CNA (Certified Nursing Assistant) or HHA (Home Health Aide) certification in the state of Indiana
- Reliable transportation
- Valid driver's license
- Physical ability to perform the duties of the role
- Strong time management skills
- Ability to work independently
- Ability to be part of a supportive team
