Job Summary
Provide direct patient care to patients under the direction of the RN and according to the Hospice Aide/Homemaker Plan of Care. Assist patients with safe transfers and ambulation per agency policy while practicing accepted infection control principles. Report and document changes and pertinent information to ensure continuity of care, and submit all documentation timely. Promote positive, supportive, respectful communication with patients, families, and other employees. Ensure overall compliance with local, state, and federal laws, Medicare regulations, and established personnel policies. Perform other duties as required. Full-time employment with competitive hourly pay, medical, dental, and vision insurance, paid time off, and tuition reimbursement. Current CNA license and at least one year of hospice or nursing assistant experience required.
Required Qualifications
- High School Diploma preferred or GED
- Must have an unencumbered certification according to the applicable Nurse Aide Registry
- Current CNA license
- At least one year experience as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice or long term care facility
- Demonstrates interest in the welfare of ill and elderly; a heart for providing compassionate care
- High integrity, including maintenance of confidential information
- Must have current driver's license, reliable transportation, and provide ongoing valid and current auto liability insurance
Desired Qualifications
- EMR experience a plus
