Hospice Aide - CNA

Provide direct patient care to patients under the direction of the RN and according to the Hospice Aide/Homemaker Plan of Care. Assist patients with safe transfers and ambulation per agency policy while practicing accepted infection control principles. Report and document pertinent information and care provided to ensure continuity of care, and submit all documentation timely. Promote positive, supportive, respectful communication to patients, families, and other employees. Ensure overall compliance with local, state, federal laws, Medicare regulations, and established personnel policies. Maintain confidentiality and demonstrate interest in the welfare of ill and elderly individuals. Requires current CNA license, one year experience in hospice or long-term care, EMR experience, and a valid driver's license with auto liability insurance. Full-time hourly position with medical, dental, vision, PTO, 401k matching, tuition reimbursement, and company car for qualifying individuals.
