Hospice Aide - CNA

Provide direct patient care to patients under the direction of the RN according to the Hospice Aide/Homemaker Plan of Care. Assist patients with safe transfers and ambulation per agency policy while practicing accepted infection control principles. Report and document pertinent information and care provided to ensure continuity of care, submitting all documentation timely. Promote positive, supportive, respectful communication with patients, families, and other employees. Ensure overall compliance with local, state, and federal laws, Medicare regulations, and established personnel policies. Perform other duties as required.
