Job Summary
Conduct personal care and home maintenance activities for hospice patients as directed by health professionals. Make scheduled visits according to assessed needs, notify patients of arrival times, and sign in logs at each location. Communicate patient condition changes to Registered Nurses and coordinate with Interdisciplinary Teams. Document services accurately and submit daily reports using required technology. Lift and move patients up to 50 pounds while maintaining body mechanics standards. Work full-time Monday through Friday from 8am to 5pm with alternating weekends.
Required Qualifications
- Must be able to lift and/or move up to 50 pounds and push/pull up to 250+ pounds
- Walk, climb stair or ladders
- Stand on feet for extended periods of time
- Maintain your required licenses, certifications and mandatory skill updates
- Comply with all policies, local, state and federal laws and regulations
