Hospice Aide

Hospice Aide (CNA) full-time covering the Vernon Hills/Des Plaines area in IL, providing in-home hospice care under RN delegation and physician orders. Responsibilities include verifying and implementing the Plan of Care during visits, communicating changes to the RN Case Manager and other team members, ensuring patient safety, teaching self-care to patients and families, suggesting revisions to the care plan when issues arise, documenting medication regimens, and collaborating with the RN Case Manager during business hours. Requires HS diploma or GED, 1 year of Hospice Aide or Nursing Assistant experience, a state-approved Hospice Aide training program with competency evaluation, 50% travel, and a valid driver’s license with insurance and reliable transportation; base pay range and benefits are provided by Elara Caring.
