Job Summary
Provide skilled, on-call nursing care for hospice patients during evenings, nights, and weekends (5pm–8am; weekends/holidays). Independently assess patients, manage symptoms, support emotional needs of patients and families, update and implement plans of care, communicate changes to the interdisciplinary team, and ensure accurate clinical documentation. Requires licensed RN or LVN in good standing; prior hospice or home health experience preferred; strong clinical judgment, excellent communication, and reliable transportation. Per diem/on-call schedule with after-hours coverage.
Required Qualifications
- Current RN or LVN license in good standing
- Hospice or home health experience preferred
- Strong clinical judgment and ability to work independently
- Excellent communication and problem-solving skills
- Compassionate presence in emotionally sensitive situations
- Valid driver’s license
- Reliable, insured vehicle in good working condition (per state and organizational requirements)
