Job Summary
Provide after-hours hospice nursing care for patients during evenings, nights, and weekends (5:00 PM start through 8:00 AM with weekend/holiday coverage). Responsibilities include responding to after-hours patient calls, performing skilled nursing assessments and symptom management, offering emotional support and education to patients and families, updating and implementing patient plans of care, communicating condition changes to the interdisciplinary team, collaborating with physicians and caregivers, and maintaining accurate clinical documentation while upholding hospice policies and regulatory standards. Requires independent clinical judgment and the ability to work with limited supervision. Licensure as RN or LVN in good standing is required.
Required Qualifications
- Current RN or LVN license in good standing
- Hospice or home health experience preferred
- Strong clinical judgment and ability to work independently
- Excellent communication and problem-solving skills
- Compassionate presence in emotionally sensitive situations
- Valid driver’s license
- Reliable, insured vehicle in good working condition (per state and organizational requirements)
