Hospice Afterhours RN/LVN (per diem/oncall)

After-hours Hospice LVN/RN providing skilled nursing care during evenings, nights, and weekends (5:00 PM–8:00 AM; weekends/holidays as assigned) with independent assessment, symptom management, and emotional support for patients and families. Works with interdisciplinary team to update plans of care, complete timely documentation, and maintain hospice standards. Requires an active RN or LVN license; valid driver’s license and reliable vehicle often required; on-call/per diem schedule.
