Hospice After-Hours RN (Registered Nurse)

Provide skilled nursing care in a patient's home as directed by medical director/attending physician; manage resource applications to meet patient needs; support chart reviews and patient phone calls from home during low after-hours needs; observe, assess, and develop individualized care plans; evaluate treatment against plan of care; determine level-of-care changes including after-hours, Routine, General Inpatient, Respite and Continuous Care; administer medications as prescribed; prioritize visits for high-risk patients; collaborate on plan of care updates; educate patient and family using teach-back; complete clinical documentation per protocol and Medicare guidelines; ensure regulatory compliance and coordination with multi-disciplinary team.
