Hospice Admissions RN

Conduct clinical assessments of new referrals from hospitals, SNFs, and rehabs while collaborating with the Practice Ambassador to process admissions. Educate the clinical care team and referral sources on community protocols, and provide patient, family, and caregiver teaching as needed. Complete admission paperwork, obtain consent, and manage EOBs with families. Participate in interdisciplinary team meetings, patient care conferences, and sales meetings with referral sources. Build and maintain relationships between hospice staff and providers, assist with state audits, and support chronic care management visits. Maintain accurate documentation and complete all required clinical trainings.
