Job Summary
Hospice Admissions Registered Nurse (RN) responsible for assessing patients for hospice eligibility, educating patients and families about hospice services, and collaborating with the Inter-Disciplinary Team to establish the initial plan of care. The role includes addressing patients' initial needs, coordinating care with the team, and enabling patients to spend meaningful time with loved ones. Strong emphasis on patient advocacy, comprehensive care planning, and ensuring high-quality, home-based hospice care. Requires an active RN license and CPR/BCLS certification, plus driving to patient locations as part of the role.
Required Qualifications
- RN license in state of work
- Two years of experience as a registered nurse, hospice experience preferred
- Current driver’s license
- Current CPR and BCLS certification
