Job Summary
Hospice Admissions Registered Nurse (RN) in the Hartford region for Trinity Health Of New England At Home. Delivers skilled nursing care in patients’ homes, creates individualized care plans, administers medications, educates patients and families, and supports recovery and independence. Role involves practicing at the top of licensure with a 1:1 patient relationship, using Epic EHR for documentation, and maintaining a 4-day schedule with possible weekend/holiday rotation. Requires CT RN licensure; BSN preferred; 1 year acute care experience; driving required. Includes a $10,000 sign-on bonus and focus on meaningful, compassionate home-based hospice care.
Required Qualifications
- Graduate of an approved nursing education program
- Licensure as a Registered Nurse in the State of Connecticut
- One (1) year experience as a professional acute care nurse
- Home care/Hospice experience preferred but not required
- Current Driver’s license and reliable transportation
