Hospice Admissions Nurse RN

Job Summary
Conduct skilled nursing assessments, plan of care initiation, and symptom management for patients and families as a member of the interdisciplinary team. Provide health care instructions, counsel on nursing needs, and coordinate care with physicians, case managers, and community health personnel. Function as a liaison during the admission process, maintain accurate clinical notes, and report abuse or neglect as a mandatory reporter. Participate in on-call duties, attend interdisciplinary meetings, and supervise ancillary personnel when assigned. Requires an RN license in good standing with at least one year of recent experience, with hospice experience being mandatory.
Required Qualifications
- Hospice Experience
- RN License
- Graduate of an accredited school of nursing
- Registered Nurse, with a License in good standing to practice in the state of Ohio
- Minimum of one (1) year of experience as a professional nurse within the last three (3) years
- Baccalaureate degree in nursing
- Two (2) years' experience, at least one of which is in the area of public health, home care, or hospice nursing
- Licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
- Visual acuity and hearing to perform required nursing skills
- Able to lift, position and/or transfer patients
- Able to lift supplies and equipment
- Considerable reaching, stooping bending, kneeling and/or crouching
- Excellent observation skills
- Excellent verbal and written communication skills
- Problem solving skills
- Basic math skills
- Nursing skills per competency checklist
- Prolonged or considerable walking or standing
- Satisfactory references
- Criminal background checks by regulation
Desired Qualifications
- Two (2) years' experience, at least one of which is in the area of public health, home care, or hospice nursing
