Hospice Admissions Nurse RN

Job Summary
Conduct skilled nursing assessments, planning, and care for patients and families on an on-call basis consistent with hospice policies. Collaborate with the interdisciplinary team, physicians, and caregivers to complete patient care planning at admission onset and provide symptom control interventions. Write and initiate plans of care, evaluate patient status, and revise plans as needs change. Administer medications, counsel families on nursing needs, and teach self-care techniques. Coordinate equipment arrangements, supervise ancillary personnel, and participate in interdisciplinary meetings and quality improvement teams. Maintain accurate clinical notes and report abuse or neglect per state regulations.
Required Qualifications
- Hospice Experience
- RN License
- Graduate of an accredited school of nursing
- Registered Nurse, with a License in good standing to practice in the state of Ohio
- Minimum of one (1) year of experience as a professional nurse within the last three (3) years
- Baccalaureate degree in nursing
- Two (2) years' experience
- At least one of which is in the area of public health, home care, or hospice nursing
- Licensed driver
- Automobile that is insured in accordance with state/or organization requirements
- Automobile in good working order
- Prolonged or considerable walking or standing
- Able to lift, position and/or transfer patients
- Able to lift supplies and equipment
- Considerable reaching, stooping bending, kneeling and/or crouching
- Visual acuity and hearing to perform required nursing skills
Desired Qualifications
- Two (2) years' experience, at least one of which is in the area of public health, home care, or hospice nursing
