Job Summary
Conduct comprehensive assessments of patients referred for hospice services to determine eligibility and appropriate care needs. Coordinate admissions while providing education and counseling to patients and families regarding end-of-life care options, advance care planning, and available resources. Assess and manage patient symptoms, obtain medical records and vital signs to support eligibility, and maintain accurate documentation of assessments, care plans, and communications. Collaborate with interdisciplinary teams including physicians and social workers to ensure a coordinated plan of care, offering emotional support during crises or acute symptom exacerbations. Protect patient confidentiality and participate in training, business meetings, and mentoring new staff. Complete assigned continuing education by the completion date. This role requires a current RN license with a minimum of two years of clinical experience in hospice or palliative care, excellent communication skills, and the ability to sit for extended periods. Occasional travel to different locations may be required.
Required Qualifications
- Current RN license in state of practice
- Minimum 2 years of experience of clinical nursing experience, in hospice or palliative care
- Excellent communication and interpersonal skills
- Compassionate, empathetic, and patient-centered approach to care
- Working knowledge of computer software programs
- Strong assessment and critical thinking skills, with the ability to make sound clinical judgements
- Ability to sit for extended periods
- Occasional travel to different locations may be required
