Job Summary
Complete hospice admissions and patient visits, including orienting new patients, assisting with legal paperwork, and conducting thorough nursing assessments. Establish hospice eligibility and diagnosis as appropriate while traveling up to 150 miles per shift to meet patients in private homes, assisted living facilities, and hospitals. Work 10-hour shifts, four days a week (Thursday through Sunday, 12 PM to 10:30 PM), requiring reliable transportation and a valid driver's license. Minimum one year of hospice, SNF, or hospital experience is required.
Required Qualifications
- 10-hour shifts, 4 days a week (Thursday-Sunday 12PM-10:30PM)
- willing to travel up to 150 miles per shift
- willing to travel throughout the Bay Area
- Minimum 1 year of hospice experience, SNF, or hospital experience
- Ability to establish both hospice eligibility and diagnosis when patient is appropriate
- Willingness to travel to meet our patients where they live (private homes, assisted living facilities, hospitals, etc.)
- Reliable transportation
