HOSPICE ADMISSION REGISTERED NURSE (RN license required)

Job Summary
Hospice Admissions Registered Nurse responsible for evaluating and admitting patients under LCD guidelines, initiating and developing individualized care plans, coordinating patient care services, documenting comprehensive assessments at the point of care, and participating in on-call rotations and monthly staff meetings. Must hold an RN license, have at least one year of clinical nursing experience, be able to deliver care in patients' homes, complete required documentation, be willing to travel, and possess strong communication, organization, and computer skills. A $10,000 sign-on bonus is offered for the full-time role.
Required Qualifications
- Current RN licensure within state of branch location
- One year of clinical nursing experience
- Nursing education from an approved school of nursing
- Ability to provide care in home setting including documentation of initial patient visit
- CPR certification
- Valid driver's license
- Willingness to travel
- Experience with computer applications (Windows, Word, Excel)
- Continuing education minimum 12 hours annually
