HOSPICE ADMISSION REGISTERED NURSE (RN license required)

Job Summary
Evaluate and admit patients according to Local Coverage Determinations (LCD) Guidelines, then initiate and develop an individualized plan of care for each patient. Coordinate necessary patient care services and document a comprehensive assessment at the point of care. Work independently in a home setting, accepting assigned travel and working conditions while participating in on-call rotation and monthly staff meetings. Maintain current RN licensure with one year of clinical experience, CPR certification, and twelve hours of annual continuing education. Hold a valid driver's license and possess strong communication and time management skills.
Required Qualifications
- RN license
- current RN licensure within state of branch location
- one year of clinical nursing experience
- Successfully completed nursing education from an approved school of nursing
- Must accept travel and working conditions as assigned
- CPR certification
- Minimum of twelve hours continuing education annually
- valid driver's license
- Background checks/drug-free workplace
