Job Summary
Hospice Admissions Registered Nurse position requiring an RN license with at least one year of clinical nursing experience. Responsibilities include evaluating and admitting patients per LCD guidelines, initiating and developing individualized care plans, coordinating patient care services, documenting assessments at the point of care, and participating in on-call rotations and monthly staff meetings. Must be able to work in home settings, travel as assigned, and work independently with strong communication, organization, and time-management skills. Required Qualifications
- RN license required
- one year of clinical nursing experience
- current RN licensure within state of branch location
- nursing education from an approved school of nursing
- CPR certification
- valid driver's license
- able to travel and work in home setting
- strong communication, organization, and time management skills
- proficient computer skills (Windows, Word, Excel)
- continuing education (12 hours annually)
