Job Summary
Conduct comprehensive assessments and initial education for patients and families entering hospice care. Collaborate with physicians to determine eligibility based on prognosis, obtain required orders, and admit patients while securing informed consent. Administer immediate symptom management treatments and teach caregivers essential skills including pain control, medication administration, and vital sign monitoring. Develop the initial interdisciplinary plan of care, coordinate necessary equipment delivery, and manage the care plan until transition to Nurse Case Management. Participate in IDG conferences to ensure continuity of care and attend to patients at the time of death if occurring during visits. Uphold compliance with agency policies and procedures.
Required Qualifications
- Graduate of Diploma or Associate degree program in nursing
- Licensed to practice in Massachusetts
- Minimum of one year in an acute care setting or comparable work experience
Desired Qualifications
- BSN
- Minimum of one year oncology and home care experience
- Minimum of one year Hospice experience
