Job Summary
Hospice Account Executive / Transition Care Coordinator in Peoria, IL territory, responsible for generating referrals and admissions, building relationships with physicians, nursing homes, hospitals, discharge planners, and case managers, delivering educational in-services, and executing marketing/territory initiatives. You will document activity and travel within your local service area, participate in evening/weekend on-call admissions, and collaborate with the Patient Care Manager and interdisciplinary team to support seamless patient transitions. Requires a Bachelor’s degree and relevant healthcare or sales experience (3+ years), excellent communication, negotiation, and public relations skills, ability to market to referral sources and communities, strong organizational skills, and a valid driver’s license. Scheduling is Mon-Fri with 8-hour shifts in the local service area, and coverage includes travel within the territory.
Required Qualifications
- Bachelor’s degree and relevant healthcare experience or knowledge preferred
- 3+ years of sales experience, ideally in a healthcare setting
- A demonstrated track record of building meaningful customer relationships that drive results
- Excellent communication, negotiation, and public relations skills
- Ability to market effectively to referral sources, customers, and the broader community
- Comfort working within an interdisciplinary team environment
- Strong organizational skills and the ability to manage a territory independently
- Valid driver’s license and willingness to travel within your local service area
- Professional appearance and demeanor at all times
