Job Summary
Hospice Access RN (32 hours/week, evenings) travels to see patients in the Portland Metro area and Clark County (WA). In the Field role, the RN visits patients in homes or facilities to present the hospice program and conduct eligibility assessments, coordinating care with Hospice MD, family and facility caregivers, and the interdisciplinary team. In the Triage role, RNs answer calls from patients and families from the Providence Home Services building in NE Portland, coordinating a whole-person plan of care per the nursing process and Nurse Practice Act. This position supports patient-focused, whole-person care within Providence Home Services Oregon, with base pay in the range shown and opportunities for shift differentials and bonuses.
Required Qualifications
- Oregon RN license upon hire
- Washington RN license within 180 days
- National Provider BLS - American Heart Association
- Criminal background check required
- 2 years of Acute care and/or hospice, home health, or long-term care RN experience
Desired Qualifications
- Bachelor's Degree in Nursing
- Call Center experience
