Job Summary
Hospice Access RN travels to see patients in the Portland Metro and Clark County area, working 32 hours per week in in-person field/home settings. The role involves visiting patients in their homes or facilities to present the hospice program, conduct eligibility assessments, and coordinate care with Hospice MD, family, and the interdisciplinary team. In the Triage role, RNs answer calls from patients and families from the Providence Home Services building in NE Portland. Requires Oregon RN licensure at hire and Washington RN licensure within 180 days, along with BLS. Preferred qualifications include a Bachelor's in Nursing and Call Center experience. Required Qualifications
- Education sufficient for licensure
- Oregon Registered Nurse License upon hire
- Washington Registered Nurse License within 6 months
- National Provider BLS - American Heart Association upon hire
- Criminal background check required
- 2 years of Acute care and/or hospice, home health, or long-term care RN experience
