Job Summary
Hospice Access RN component: travel to see patients in the Portland Metro and Clark County area; 32 hours/week with 1100-1500 start; in Field role visit patients in homes to present hospice program and conduct eligibility assessments, coordinating care with Hospice MD, family and IDT; in Triage role, answer calls from patients and families from the Providence Home Services building in NE Portland; join Providence Home Services Oregon with patient-focused, whole-person care and collaborative team environment.
Required Qualifications
- Education sufficient for licensure
- Oregon Registered Nurse License
- Washington Registered Nurse License
- National Provider BLS - American Heart Association
- Background check
- 2 years of Acute care and/or hospice, home health, or long-term care RN experience
