Job Summary
Hospice Access RN position traveling to see patients in the Portland Metro and Clark County (WA) area. Part-time (32 hours/week) with shift 1100–1500. In-field RN visits patients at home or facilities to present the hospice program, conduct eligibility assessments, and coordinate care with Hospice MD, family and IDT; Triage RN answers calls from Providence Home Services building in NE Portland. Roles include field visits and triage responsibilities, focusing on whole-person care within the hospice program, following nursing processes and licensure requirements. This role is on-site with travel between Oregon and Washington within the specified service area.
Required Qualifications
- Oregon Registered Nurse License
- Washington Registered Nurse License within 6 months
- National Provider BLS - American Heart Association
- Criminal background check per role duties
- 2 years of Acute care and/or hospice, home health, or long-term care RN experience
Desired Qualifications
- Bachelor's Degree Nursing
