Job Summary
Hospice Access RN provides in-home and triage hospice care in the Portland Metro and Clark County area. The role involves a 32-hour workweek with a window from 1100 to 1500, visiting patients in their homes as part of the Field RN duties and answering patient/family calls from the Providence Home Services building in NE Portland as part of the Triage RN role. Responsibilities include presenting the hospice program, completing eligibility assessments, coordinating care with Hospice MD, family and facility caregivers, and the interdisciplinary team (IDT), following the nursing process and Nurse Practice Act. The position requires Oregon RN licensure with WA RN licensure within 180 days of hire, and BLS certification. Required Qualifications
- Education sufficient for licensure
- Oregon Registered Nurse License
- Within 6 months (180 days) of hire: Washington Registered Nurse License
- National Provider BLS - American Heart Association

- 2 years of Acute care and/or hospice, home health, or long-term care RN experience
- Bachelor's Degree Nursing (preferred)
- Call Center experience (preferred)
