Job Summary
Hospice Access RN in the Portland Metro area performing in-home visits and triage responsibilities. The role includes visiting patients in their homes or facilities to present the hospice program, conduct eligibility assessments, coordinate care with Hospice MD, family, and interdisciplinary team, and responding to calls from the Providence Home Services building for triage. The position is part-time (32 hours/week) with shifts from 1100 to 1500 and requires travel to patient sites in Portland Metro and Clark County (WA). Licensure in Oregon with Washington licensure within 180 days, and BLS certification are required. The role emphasizes whole-person care and collaboration with patients, families, and care teams within Providence’s patient-centered culture.
Required Qualifications
- Oregon Registered Nurse License (upon hire)
- Washington Registered Nurse License (within 6 months of hire)
- National Provider BLS - American Heart Association (upon hire)
- Criminal background check (required)
- 2 years of Acute care and/or hospice, home health, or long-term care RN experience
