Hospice Access RN

Hospice Access RN role traveling to see patients in the Portland Metro and Clark County (Washington) area. Part-time 32 hours/week with shift from 1100 to 1500. The Hospice Access Field and Triage RN will perform nursing duties per the nursing process, coordinate a whole-person plan of care, and conduct eligibility assessments in patients' homes or via triage from the Providence Home Services building in NE Portland. In-field visits involve presenting the hospice program and coordinating care with Hospice MD, family, facility caregivers, and the interdisciplinary team (IDT). In triage, RNs answer calls from patients and families. The position emphasizes patient-focused, whole-person care and collaboration within Providence Home Services Oregon, with an on-site work arrangement in Portland, Oregon. Requirements include Oregon RN licensure at hire, Washington RN licensure within 180 days, BLS certification, and a background check; preferred qualifications include a Bachelor’s in Nursing and Call Center experience.
