Job Summary
Hospice Access RN provides in-field hospice eligibility assessments and care coordination, traveling to patient homes in the Portland Metro and Clark County, WA area. The role combines field visits (home or facility visits to present the hospice program and coordinate care with the Hospice MD, family, and the interdisciplinary team) and triage duties (answering patient/family calls from the Providence Home Services building in NE Portland). The position supports a whole-person plan of care following the nursing process and Nurse Practice Act, with shift hours around 1100–1500 and a 32-hour work week, and is part of Providence Home Services Oregon. Required Qualifications
- Oregon Registered Nurse License (active)
- Washington Registered Nurse License (within 180 days)
- National Provider BLS - American Heart Association
- Criminal background check per Providence Health and Services-OR requirements
- 2 years of Acute care and/or hospice, home health, or long-term care RN experience
- Bachelor's Degree Nursing (preferred)
- Call Center experience (preferred)
