HOS Medical Social Worker

Job Summary
Provide coordinated in-home psychosocial assessments and counseling to clients of all ages; perform admissions evaluation and develop care plans addressing psychosocial, financial, environmental and community needs; collaborate with interdisciplinary team to coordinate patient care; accept assignments consistent with education and competence; communicate effectively with clients, families, team members, and other healthcare professionals; participate in case conferences/teams; maintain continuing education and competency; travel to patient homes and office/support locations; ensure documentation and eligibility standards are met; possess and maintain required licensure and demonstrate knowledge of community resources.
Required Qualifications
- MSW degree from an accredited school or Baccalaureate degree in social work (or related field) supervised by MSW; or Baccalaureate degree in psychology/sociology related to social work with MSW supervision; current state licensure as required; current driver’s license; 1 year of social work experience in a healthcare setting; knowledge of community resources; strong interpersonal skills; ability to travel to patient homes and office/support centers; ability to work flexible schedule.
