Job Summary
Provide patient care in the patient's place of residency, adhering to federal, state, and local standards and the Director of Nursing's guidelines. Perform duties assigned by the Case Manager including baths, oral hygiene, nail care, foot care, skin care, pressure relief, dressing, feeding, and transferring patients. Take and record vital signs in the EMR, assist with activities of daily living, and position patients with proper body alignment. Notify licensed personnel for values outside normal parameters or patient pain complaints. Document activities of daily living and prepare clinical notes within 24 hours. Maintain 12 hours of annual education, comply with supervisory visits every 14 days, and ensure patient safety while working as part of the interdisciplinary team.
Required Qualifications
- CNA certificate obtained by: Completion of a CNA course Certified by State Department of Health Services
- Maintains 12 hours of annual education
- Adheres to dress code, appearance is neat and clean
- Maintains patient confidentiality
- Reports to work on time and as scheduled
- Completes in-services and returns in a timely fashion
- Attends annual review and department in-services, as scheduled
- Represents the organization in a positive and professional manner
- Communicates the mission, ethics and goals of the organization
- Demonstrates effective time management and organizational skills
- Maintains regulatory requirements
