Job Summary
Engage families entering shelter early to provide comprehensive service coordination, ensuring they build strengths and address needs for stable permanent housing. Build relationships to set goals, facilitate collaboration with community resources, and serve as a liaison between families and the Coordinated Entry system. Conduct assessments using tools like FAST, CANS, and ANSA to identify strengths and needs, while advocating for timely service access and maintaining records in tracking systems. Work directly with families to revise plans, check progress, and empower self-advocacy, adjusting schedules to accommodate family needs.
Required Qualifications
- Master's degree in a human services field, plus three years of experience working in the human services field performing direct practice
- Bachelor's degree in a human services field, plus five years of experience working in the human services field performing direct practice
- Any equivalent combination of education and experience which meets the required knowledge, skills, and abilities
- Act 33, 34 and FBI clearances
- Valid driver's license and access to a reliable vehicle, or other reliable form of transportation to locations around the county
Desired Qualifications
- Has an understanding of and/or willingness to learn the principles of Harm Reduction, Trauma-Informed Care, and Housing First, as well as experiencing implementing services guided by these principles
