Job Summary
Conduct initial screenings to assess service needs and facilitate appointments for referrals and housing solutions. Act as a liaison between clients and supportive services, shelters, and community agencies while locating, verifying, and transporting individuals to pre-arranged emergency shelters. Enter and maintain client data in the Homeless Management Information System, complete triage forms, and escalate critical situations to management. Support callers through a strength-based approach to problem-solve housing stability and refer clients to specialized providers as necessary. Navigate Santa Clara County to walk through encampments and waterways, drive during traffic hours, and clean transportation vehicles. Work 7 days a week, 10 hours a day, within the county's Coordinated Entry System to end homelessness.
Required Qualifications
- High school diploma or Equivalent
- Must have valid California driver's license
- Ability to be insured through the company
- Ability to read and interpret documents
- Ability to write reports
- Ability to talk and collaborate with staff, other organizations, and clients
- Ability to navigate through Santa Clara county
- Ability to stay seated for long periods of time in the office and in vehicles during work hours
- Ability to walk through homeless encampments and water ways
- Specific vision abilities required by this job include close vision, color vision, and the ability to adjust focus
Desired Qualifications
- Personal or lived experience with the criminal/legal system, housing instability, social services, or other systemic challenges is valued
