Job Summary
Provides therapeutic social work services for patients, residents, and families under physician supervision; assesses biopsychosocial functioning, crisis intervention, and planning of care; develops and revises care plans; conducts short-term individual counseling, family education, and advance directives preparation; ensures thorough medical record documentation; coordinates with an interdisciplinary team; facilitates discharge planning and linkage to community resources; educates families on suicidality, diagnoses, and plan of care; supports patients’ financial, legal, and healthcare decisions; maintains collaboration with referral sources and care teams.
Required Qualifications
- Masters Degree, Social Work (Required)
- Clinical internship in a hospital or residential setting or two years clinical experience (Preferred)
- CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) within 30 Days(Required)
- Licensed Clinical Social Worker (LCSW) - Virginia Department of Health Professions (VDHP) (Preferred)
- Valid Drivers License Required
