Job Summary

Coordinate and case manage total care for Home Care and Hospice patients, performing initial and ongoing physical, psychosocial, and environmental assessments. Develop individualized plans of care including nursing diagnoses, interventions, goals, referrals, and discharge strategies while involving patients and families in decision-making. Conduct home visits across various areas during inclement weather, performing technical procedures within licensure standards and obtaining signed consents. Supervise Home Health Aides, complete documentation of vital signs and interventions, and collaborate with multidisciplinary teams to ensure continuity of services. Maintain a case load of 25 visits per week and participate in mandatory on-call rotations and weekend shifts.

Required Qualifications

  • Diploma or Associates Degree in Nursing
  • Licensed to practice as a Registered Nurse in the State of New Jersey
  • Valid driver's license

Desired Qualifications

  • One year medical/surgical experience in hospital setting
