Job Summary
Coordinate and oversee the interdisciplinary plan of care for an assigned caseload of hospice patients, ensuring they receive compassionate, high-quality care wherever they call home. Develop individualized care plans, manage symptoms, and educate patients and families while collaborating with physicians and the interdisciplinary team. Supervise Hospice Aides and LPNs, maintain accurate electronic medical record documentation, and participate in quality improvement initiatives. Deliver exceptional end-of-life care while upholding Big Bend Hospice's mission of providing comfort and dignity throughout the patient's journey.
Required Qualifications
- Current Florida Registered Nurse (RN) license
- Minimum of one year of inpatient nursing experience or previous hospice, home health, or long-term care experience
- Valid Florida driver's license
- Reliable transportation
- Current auto insurance
