Job Summary
Provide expert professional nursing care to homebound patients within a value-based care program. Assess, plan, implement, and evaluate care for patients with urgent, emergent, or potential health problems, focusing on improving clinical outcomes, care coordination, and patient experience while reducing unnecessary utilization. Collaborate with interdisciplinary teams to support chronic disease management, care transitions, preventive care, and population health initiatives; participate in department management. Requires state RN licensure, BLS certification, clinic-based nursing experience, and the ability to travel to patients’ homes. Must have a valid driver’s license with insured vehicle, and be able to perform typical home-visit duties (standing, walking, lifting, transferring patients).
Required Qualifications
- Graduation certificate from an accredited RN school of nursing
- Current state licensure
- BLS certification
- Experience in clinic-based nursing
- Effective communication with patients, families, and staff
- Ability to work compatibly with physicians and other staff
- Licensed driver with automobile insured per requirements and in good working order
- Ability to make home visits to patients/families in a variety of settings
