Home Visitor (Grant) – Community Outreach – St. Vincent Medical Center

Provide culturally sensitive home visitation services to expectant moms and families with young children, promoting positive parent-child interaction and child development skills using best practice model standards. Administer developmental assessments to monitor progress and link families to medical providers for timely care. Develop individualized family service plans, maintain accurate electronic case files, and coordinate services with community partners. This role supports the Community Outreach department at St. Vincent Medical Center within the Mercy Health system.
