Home Visiting Nurse- Family Connects (Somerset County)

Job Summary
Provide home visiting nursing for newborn families under the Family Connects program in Somerset/North Brunswick area. Conduct health assessments of mother and baby in the home, evaluate family needs across 12 physical and psychosocial domains, facilitate referrals to community resources, and document observations in real time using electronic records. Schedule and coordinate home visits, visit target hospitals to arrange visits, and collaborate with a nurse supervisor and team to develop and implement family support plans. Maintain eight open visit slots and conduct at least six home visits per week, assisting families in attending medical appointments and connecting with social services. Act as a liaison between families and community agencies, applying knowledge of parent-child interaction, child-maternal health, child development, and abuse/neglect to improve parenting supports and reduce family stress. Expect travel by personal vehicle and participation in team meetings and clinical supervision.
Required Qualifications
- A.S. degree in nursing with state-level RN license and at least one year of relevant nursing experience or B.S. degree in nursing with current RN license, or B.A. degree in another field with current RN license
- Current CPR certification
- Valid RN license in New Jersey (or compact licensure)
- Ability to work in a team with clinical and non-clinical staff and culturally diverse populations
- Computer proficiency with Microsoft Office programs (Windows, Outlook, Word, Excel, PowerPoint)
- Valid driver's license with ability to provide proof of ongoing automobile insurance and willingness to travel locally/statewide
