Job Summary
Family Connects Home Visiting Nurse provides in-home visits for families with newborns in Ocean County, following the Family Connects protocol. Responsibilities include conducting health and physical assessments of mothers and babies, assessing 12 domains of physical and psychosocial wellbeing, making and coordinating referrals to community resources, documenting in real time, scheduling visits, coordinating with hospitals and team members, developing family support plans, and supporting nurses in clinical development plans. Requires collaboration with a nurse supervisor, travel to client homes, and the ability to work with culturally diverse populations; must maintain visits, participate in team meetings, and assist families in accessing healthcare, social services, and other resources.
Required Qualifications
- RN license (state-level license)
- Current CPR certification
- Education: A.S. degree in nursing with RN license (state-level) or B.S. degree in nursing with RN license or B.A. degree with RN license
- Ability to work in a team with clinical and non-clinical staff
- Valid driver's license with ongoing automobile insurance
- Experience in non-profit, maternal-child health and education preferred
- Computer proficiency with Microsoft Office suite
