Home Visiting Nurse- Family Connects (Middlesex County)

Job Summary
Conduct home visits to support newborn families in Middlesex County by assessing physical and psychosocial wellbeing across 12 domains, including depression, substance abuse, and domestic violence. Perform health and physical assessments of mothers and babies, document observations in real time via computerized systems, and maintain eight open visit slots with at least six visits per week. Establish trusting relationships to create family support plans, facilitate referrals to community agencies, and assist parents in attending medical and social service appointments. Act as a liaison between families and external resources while attending weekly team meetings and clinical supervision. Maintain valid driver's license and ability to travel locally and statewide for visits.
Required Qualifications
- A.S. degree in nursing with state-level RN, and at least one year of relevant nursing experience
- B.S. degree in nursing with current RN license
- B.A. degree in another field with current RN license
- Current CPR certification
- Valid Driver's license
- Ability to provide proof of ongoing automobile insurance coverage
- Computer proficiency with Microsoft programs including Windows, Outlook, Office 2007, Access, Excel, Power Point, Publisher and Word
- Strong interpersonal & oral/written communication skills
- Ability to occasionally lift up to fifty pounds of materials or equipment
