Job Summary
Remote Home Visit Coordinator responsible for outbound calls to educate patients about the Home Visit Program, build rapport, and coordinate care to reduce re-hospitalizations. Schedule appointments by geographical area, enter and assign patients to providers using multiple systems (SMED, hospital systems, Microsoft platforms), and submit paperwork to PCP offices while maintaining accurate records. Maintain follow-up calls to identify patient needs or support opportunities, communicate with referral sources and community partners, and meet outreach and scheduling goals through persistent relationship-building in a fast-paced, phone-heavy remote environment.
Required Qualifications
- Minimum 1 year of experience scheduling patient appointments
- Minimum 1 year of experience in a healthcare setting
- High School Diploma or GED
- Comfortable spending several hours per day on outbound calls
- Strong multitasking and organizational skills
- Friendly, patient, and confident phone presence
- Self-motivated with the ability to work independently in a remote environment
- Creative problem-solver who can think outside the box to connect with patients and drive results
- Experience navigating multiple computer systems and platforms simultaneously
- Excellent written and verbal communication skills
- Bilingual (English/Spanish) preferred
