Job Summary
Conduct home visits to assess patient environments for dialysis suitability and safety compliance with CMS regulations. Initiate practice as a SHC coordinator of patient care, acting as a teacher and advocate to ensure informed consent and promote patient independence. Identify professional development activities including continuing education, quality assessment, and clinical application of research findings while addressing ethical challenges independently. Maintain current state RN licensure, Nephrology experience with dialysis, and a valid driver's license for home visits.
Required Qualifications
- One (1) year of registered nurse experience
- One (1) year of experience as a Nephrology Nurse with dialysis experience required
- in peritoneal dialysis, home hemodialysis, or a combination of both
- Graduation from an accredited nursing school or equivalent
- Current, active, unencumbered state RN licensure
- Current Healthcare Provider certification (CPR) required
- Competent and in-depth knowledge regarding the use of Electronic Health Records
- Ability to organize, prioritize and complete projects independently
- Valid state driver's license and reliable transportation for home visits
